How Do I.... Register With The Practice?

New patients will find details on registration at Reception. If you have not previously been registered under the NHS you will be asked a number of questions and proof of identity will be required.
Every patient is registered with the York Medical Practice and not an individual Doctor. However, we shall note a patient's preference of practitioner on the registration details. When registering you will be asked to complete a form about your health and we encourage you to accept a new patient health screening.

How Do I.... Change My Details?

It is important that you tell us of any change of name, address or telephone number. In an emergency this could be vital. We can only provide our services within our catchment area. If you move away from the Practice area, we regret that you will have to register with a GP in your new locality.
